Successful Practices in Accountable Care: Coal Country Community Health Center

From 2011 to today, two organizations devoted to serving the health of the community developed and grew into a strong partnership through transparency and collaboration. Coal Country Community Health Center and Sakakawea Medical Center are a model of how innovative partnerships can improve quality of care for a rural community and lower total costs.
