Successful Practices in Accountable Care: Piedmont Health

Piedmont Health Services (PHS) is a Community Health Center in North Carolina with twelve locations across the state, two of which are designated as PACE service sites. Their experience developing and implementing the PACE program has prepared them to take on additional elements of accountable care. The Program for All-Inclusive Care for the Elderly (PACE) allows health centers to provide coordinated services for a vulnerable population while learning to accept and manage risk-based contracts. This program as a whole, and the lessons learned in its development, serve as examples for other health centers considering value based payment.
