Arizona Alliance for Community Health Centers (AACHC) & Arizona Council of Human Service Providers (ACHSP) 

The Arizona Alliance for Community Health Centers (AACHC) is a member organization of 25 community health centers most of which also participate in its affiliate, Collaborative Ventures Network (CVN), a grantee of a HRSA Health Center Controlled Network and sponsor of a clinically integrated network. Arizona’s participation in the Delta Center is an extension of CVN’s existing work with another RWJF initiative, The National Safety Net Advancement Center (SNAC), through which they have begun working to advance delivery and payment reform for Arizona’s safety net providers. AACHC/CVN are collaborating with the Arizona Council of Human Service Providers (ACHSP), a member organization of 100 human service providers, on integrating physical and behavioral health for value-based payment and care.

Recently, AACHC has been working to develop an APM proposal to reform state PPS rate setting for FQHCs. If CMS accepts the proposed State Plan Amendment, the new APM will be effective as early as October 2018. ACHSP’s primary focus at this time is to provide leadership and influence payment reform and care integration.  Delta Center support will help Arizona’s efforts in continuing to advance integrated value-based care and payment, including implementation of promising practices for the identification, capture, analysis and reporting of physical and behavioral health clinical measures that support population health management.
