The Delta Center for a Thriving Safety Net held a fourth convening for state primary care associations and behavioral health state associations participating in the State Learning & Action Collaborative in Princeton, NJ on October 21-22, 2019.

DAY 1
Welcome
Keynote Address: Behavioral Health and Primary Care Integration – Making it Real
Dr. Parinda Khatri with Cherokee Health Systems shares the path to integration at Cherokee and highlights their barriers, lessons learned, and keys to success relevant to patients, payment, practice, and policy.
Keynote Address: Toward a Vision of Integrated Care: Putting Health Care in Context
Dr. Rachel Solotaroff and Freda Ceaser, MSW, from Central City Concern will share a model to segment and support populations affected by homelessness and will discuss barriers and opportunities to develop integrated models to support these populations.

Lessons Learned: Poster Sharing Session
Grantees created posted to learn from peers. Posters are available on the For Grantees section of the website.
Payment Open Space
Discussion and facilitation of topics in payment including demonstrating ROI to stakeholders, reinvestment in the safety net, pulling together a persuasive data story to make the case for cost savings to payers, and any topics that emerge for discussion.
DAY 2
Integrated Technical Assistance: What’s Working
Using an interactive “fishbowl” style conversation, grantees shared what they’re learning about how best to provide technical assistance to both behavioral health and primary care members with your peers.
Deep Dive: Learner-Centered Methods for Improved TA
Revisit the “learner-centered design” methods we have used throughout the Delta Center to leverage the skills and expertise of the participants to deliver stronger, more relevant technical assistance.

Partners in Innovation: Reflection on Innovative Approaches to Accelerate the Adoption of VBP
Teams complete an after-action review on your progress in advancing value-based payment in your state and identify examples of innovative VBP models to shape goals and approach for the remainder of the Delta Center project.
