The Colorado Community Health Network (CCHN) represents Colorado’s 20 community health centers (CHCs) operating in over 200 sites. The CCHN team is most proud of their persistence through delays and state leadership changes on their APM proposal, and their work to continue to support Colorado’s CHCs on the team-based care part of the payment reform proposal despite the delays. Through the Delta Center, CCHN was able to convene their first Social Determinants of Health Learning Community.
