The Federally Qualified Health Center Urban Health Network (FUHN) is a health center coalition-led ACO in the Twin Cities area. This case study examines the origins, structure, and performance of the FUHN. In exchange for a portion of the shared savings, FUHN built a robust data analytics infrastructure with the aid of a health services and data management firm. This data infrastructure formed the foundation for valuable delivery system improvements and millions of dollars in shared savings. FUHN’s story offers lessons for health centers interested in forming or participating in health center-led ACOs or IPAs, and individual health centers pursuing shared savings arrangements on their own.
