This series of case study profiles innovative rural health system transformation models and strategies from various regions of the United States. The aim is to assist rural communities and regions to proactively envision and develop strategies for transforming rural health in the state. Research included consulting experts, interviewing key informants, and reviewing the professional and research literature to find robust and innovative models and strategies that could be replicated in rural communities.
