Mosaic Medical’s Participation in the Central Oregon CCO and Three-Layer Payment Reform

This case study examines the participation of Mosaic Medical Health Center in the Oregon Coordinated Care Organization (CCO) and in one of the most advanced health center alternative payment models in the country. Mosaic’s ability to collaborate with diverse healthcare system partners in the CCO and its robust “three-layered” payment model allowed the health center to provide more flexible and high quality care, shifting from volume-based services delivery to improving population health. Mosaic’s story offers lessons for health centers interested in strong representation of primary care in community coalitions to pursue accountable care, particularly in rural areas, and movement towards multi-layered payment models.
