The Safety Net Medical Home Initiative

The Safety Net Medical Home Initiative (SNMHI) was a national Patient-Centered Medical Home (PCMH) demonstration to guide 65 primary care safety net sites to become high-performing medical homes and improve quality, efficiency and patient experience. The initiative created a framework of eight change concepts to guide PCMH transformation and published a library of resources to aid practices in implementing a new model of care.
